1) To schedule a Moderna Covid vaccine proceed to your portal and sign in
2) Click "appointments" on the left side
3) Under "I'd like to schedule a new appointment" choose either "Covid-19 Moderna Vaccine Initial" if this will be your first or second dose of covid vaccine. If this is a booster then Choose "Covid-19 vaccine Moderna subsequent"
4) Click find appointment- you will see available appointments on upcoming Wednesday afternoons and Friday mornings- Choose the time that you would like
We are very concerned about wasting vaccines once the vials are open. Once the Moderna vial is open we have 6 hours to give all of the doses. If you need to cancel your appointment please do so as soon as possible so that we may find another patient to fill that slot.
